Welcome 2010-11 Menace Academy Players!
Registration is July 13, 2010 from 5:00 to 7:30pm at the Kum & Go Headquarters located at 6400 Westown Parkway in West Des Moines. If you are unable to attend, paperwork and your first payment are due by July 15th!
1. Paperwork can be filled out and turned in (copies of all paperwork are available below and on-site at registration)
a. Bid Acceptance Letter
b. Menace Player Form
c. Notarized Medical Release (notary will be on-site)
d.Youth Participation Form (only 1 needed per 10 years participation with the club)
e. Copy of player's Birth Certificate
f. One (1) small photo of player
2. Sample uniform sizes will be available to try on. 2010-11 Uniform Kit Information
3. Practice Gear will be available for pick-up.
4. Payment will be accepted for uniforms and Academy fees. 2010-11 Academy Fee Information
If you're experiencing problems with the online store, please contact Abby Johnson at (515) 457-6034 to pay over the phone.
**As a registered player you are responsible for full payment of Academy dues regardless of payment options and player status. If for any reason you choose not to continue the entire soccer calendar year with the Menace you are still financially responsible for fees in their entirety.**

DES MOINES AREA SELECT SOCCER COALITION (DMASSC)
Bad Standing and Player Eligibility
Any soccer player may be put into bad standing or lose player eligibility for non-payment of fees owed to ISA or an affiliate of ISA (ISA Bylaws Article II) including one of the Des Moines Area Select Soccer Coalition Member Clubs.
Penalties- Player cards may be collected by the club and withheld until the outstanding balance is cleared up with the DMASSC member club and written verification from the club is received by ISA. If the outstanding balance is not cleared up by the end of the season, select players will not be offered bids to play for another DMASSC member club until written verification is received from the club in which an outstanding balance still remains.
*January 1 and June 1 of each year a list of players with outstanding balances owed to their current member club will be submitted to ISA for sanctioning.
