REQUIRED ACADEMY REGISTRATION FORMS $150 Registration fee is due at time of registration. As a registered player you are responsible for full payment of Academy dues regardless of payment options and player status. If for any reason you choose not continue the entire soccer calendar year with the Menace you are still financial responsible for fees in their entirety.
Player Registration Form Any player who makes the Academy team must fill out a Player Registration Form.
Medical Release Form Any player that is a member of ISA must fill out a Medical Release form.
