Team Check-In will now take place online. Please submit league roster with photos in PDF form (or player cards in PDF form) for approval.   These are often referred to as game-day rosters or game sheets.

Teams must submit a copy of their roster with player photos for approval via online check-in form. 

Once reviewed, you will be emailed a copy of your roster with an approved watermark.

Please have one copy of this printer in color for each game you may play.  (Four-five copies in case of playoff/finals).

These rosters will be reviewed prior to each game for team check-in.

Teams with guest players need to present (via email):
>> Guest-player form (if required by your association); Maximum of five guest players per team.
>> Player passes for guest players
>> Medical releases (do NOT need to be notarized) for guest players

Teams from outside Iowa must present a permission-to-travel form, unless they are affiliated with US Club Soccer.
